PatientWisdom

PatientWisdom is a secure tool that improves the experience and delivery of care. We realize that you might have questions about the best way to use PatientWisdom. Here are some pointers:

  • My Settings contains information you provided during the Sign Up process. You can update this information anytime.

  • My Stories is the place to upload information about yourself, your health, and your care. Click on any of the stories to get started. Your information will be saved as you type, but it’s important that you click on Submit for it to be available for your care team to view. You can update your stories at any time.

  • Based on information in My Stories and My Settings, PatientWisdom creates the inSIGHT Summary, an at-a-glance view in your electronic health record to help your entire care team know what’s important to you as a person. You can always update your inSIGHT summary by changing information in My Stories or My Settings.

  • To Log Out, just click on the icon at the top of the menu. You might want to bookmark PatientWisdom so it’s easy to find the next time you want to Log In.

  • If you are invited to complete an Anonymous Survey - for instance, the Communication Assessment Tool or a Community Needs Assessment - please follow the link and be sure to click on "Submit" when finished. The survey is completely anonymous: All answers are secure and confidential.

ProviderWisdom

ProviderWisdom is a secure digital tool that focuses on what matters to those who deliver care or support the people who do. Clinicians and staff share perspectives about themselves, their work, and their organization, as well as their ideas for solving problems. We distill this information into meaningful, actionable insights to drive learning, improvement, and value.

We realize that you might have questions about the best way to use ProviderWisdom. Here are some pointers:

  • My Settings contains information you provided during the Sign Up process. You can update this information anytime.

  • My Stories is the place to upload information about yourself, your work, your organization, and your ideas for improvement. Click on any of the stories to get started. Your information will save as you type, but it’s important that you click on Submit to update the Snapshot views. You can update your stories at any time.

  • Based on information in My Stories and My Settings, PatientWisdom creates Snapshots for your colleagues and for your leaders -- these at-a-glance views contain information that is helpful and appropriate to share. In the Snapshots tab, you can view what your colleagues and leaders will see. You can always update your Snapshots by changing information in My Stories or My Settings. You can also view Snapshots of your colleagues by clicking on “My Colleagues.”

  • VIEWpoint allows you to browse a graphical Reports interface to see the aggregation of what you and your colleagues are sharing in My Stories. Click on any of the topics to get started. You can navigate between the reports using the arrows at the bottom of the screen.

  • To Log Out, just click on the icon at the top of the menu. You might want to bookmark ProviderWisdom so it’s easy to find the next time you Log In.

  • If you are invited to complete an Anonymous Survey - called a Pulse Check - please follow the link and be sure to click on "Submit" when finished. The survey is completely anonymous: All answers are secure and confidential.
